Quote requests submitted on kazakLPI.com are sent by email to David Kazak at david@kazaklpi.com. They are used only to prepare life-insurance options related to cryonics funding. They are not sold, not published, and not written to a public spreadsheet.
The form asks for contact details, demographics, health and risk information, and cryonics preferences because carriers underwrite on those facts. Phone is required so we can follow up.
A hidden honeypot field and a bot check (Cloudflare Turnstile, when configured) reduce automated spam. We also apply a simple rate limit.
